Entrants will be asked to write about why they need Facelift Dentures and what current or upcoming events are making 2012 a turning point in their lives and futures. Entries will be judged by content, not by spelling or grammar. We want to know how Eagle Point Advanced Dental could positively impact your life.

Did they go back to school and now need a smile for the job market?

Are they starting a new business that requires them to interact with the public?

Finalists will be voted upon by the public to determine who should receive our SmileGift 2012. The winner’s dental treatment will be completed in spring, 2012. Entries will be reviewed and five individuals will be selected for advanced research. Eagle Point Advanced Dental will verify the facts in the essay and photos of the preliminary finalist will be required. Eagle Point Advanced Dental will also verify that the preliminary finalists are, in fact, candidates for dentures as a next logical step in their dental care. From the five preliminary finalists, three finalists will be presented to the public for voting to determine the SmileGift 2012 winner.

Entrants are asked to write a 500-words or less story about their (or the person they are nominating) train-wreck smile and their current or upcoming life changes that would be made perfect with a new smile.

Eagle Point Advanced Dental will complete a new patient exam including x-rays and regular periodontal therapy. To prepare the winner for their new Facelift Dentures, teeth will be extracted (if necessary), neuromuscular occlusion readings will be taken for optimal occlusion and impressions of their gums will be formed. A pre-denture model will be crafted to insure optimal fit and comfort. Finally, the finished Facelift Dentures will be placed in the patient’s mouth for the ultimate in smile rehabilitation.

The winner will appear in one of Eagle Point Advanced Dental’s television commercials scheduled for fall, 2012.

1. ELIGIBILITY: Contest open only to legal residents of the United States who are 21 years or older. Void where prohibited. Employees of Eagle Point Advanced Dental and their immediate family and household members, their affiliates, advertising and promotion agencies, and any entities or individuals associated with the development, fulfillment of, or supply to this contest are ineligible to enter or win. Entrants agree to be bound by the terms of these official rules and consent to use of their contact information for future communications including Eagle Point Advanced Dental advertising, promotions, surveys, contest/sweepstakes and other similar communications without compensation or permission, unless prohibited by law. Entrants that do not consent to such use of their contact information can call Eagle Point Advanced Dental (1-800-791-7333) to unsubscribe. 2. HOW TO ENTER: Simply go to the “SmileGift 2012” link below and follow the online entry instructions to register for the contest. Limit one (1) entry per person/e-mail address for the duration of the contest. Entries must be received and recorded by Eagle Point Advanced Dental (as recorded by our computer for online entries) by 11:59 p.m. Pacific Time on Monday, October 31st, 2011 to be eligible. Eagle Point Advanced Dental’s computer is the official timekeeping device for this contest. Entries will also be accepted by printing your name, complete address, phone number, e-mail address and 500-word or less essay on standard typing paper. Paper entry forms should be mailed to Eagle Point Advanced Dental, SmileGift 2012, Attn: Sammi, 217 W. Main St. PO 236 Eagle Point, OR 97524. Entries qualified upon receipt by Eagle Point Advanced Dental. Paper entry forms must be received by 5:00 p.m. PDT on Monday, October 31st, 2011 to be eligible. All entries become the property of Eagle Point Advanced Dental and will not be returned. Eagle Point Advanced Dental is not responsible for late, lost, stolen, damaged, misdirected, incomplete or illegible entries. Only one form of participation is valid. Any attempt by any participant to obtain more than one (1) entry by using multiple/different email addresses, identities or any other methods will void that participant’s entries and that participant will be disqualified.

4. AGREEMENT TO OFFICIAL RULES: Participation in the contest constitutes entrant’s full and unconditional agreement to and acceptance of these Official Rules and the decisions of Eagle Point Advanced Dental, which are final and binding. Winning a prize is contingent upon fulfilling all requirements set forth herein in a timely manner. Time is of the essence.

5. WINNING: Winner will be determined by means of a public vote whereby the public will review the details of entrants’ essays and their photos. Voting will occur between Wednesday, November 16th, 2011 and Saturday, December 31st, 2011. Potential prize winner will be notified by email, letter and/or telephone call. Potential winner will have 72 hours to respond to the notice. If no response is received within above given time frame, the potential winner will be disqualified and an alternate will be selected. Prize winner will be required to complete and sign before a Notary Public and return an Affidavit of Eligibility/Liability and Publicity Release (where legal) within fourteen (14) days of the date of notification or prize will be forfeited and an alternate winner selected. Return of prize notification as undeliverable will result in disqualification and selection of an alternate winner. Public announcement of the winner will be on Monday, January 9th, 2012 at 12:00 pm (noon) PDT.

6. PRIZES: One (1) winner will be chosen. Winner will receive preliminary assessment plus all necessary dental care needed for the placement of a lower, upper, or complete set of LifeStyle Dentures. Approximate retail value $8,000 – $15,000. Eagle Point Advanced Dental will determine exactly what is appropriate for the winner’s successful completion of denture placement. Winner is responsible for all federal, state and local taxes. Prize is not redeemable for cash; nor is it transferable or substitutable except by Eagle Point Advanced Dental with one of equal or greater value. Odds of winning depend on the total number of eligible entries received. If a winner cannot accept prize as stated, prize will be forfeited in its entirety and an alternate winner will be chosen.

7. GENERAL: Eagle Point Advanced Dental reserves the right to cancel, suspend, and/or modify the contest if any fraud, virus, bugs, non-authorized human intervention or other technical problem corrupts the administration, security, proper outcome of the contest, or submission of entries of the contest as determined in its sole discretion. In such event, Eagle Point Advanced Dental reserves the right to award the prize at random from among the eligible entries received up to the time of the impairment. Eagle Point Advanced Dental reserves the right in its sole discretion to disqualify any individual it finds to be tampering with the entry process or the operation of the contest or to be acting in violation of these Official Rules or in an unsportsmanlike or disruptive manner. Any attempt by any person to undermine the legitimate operation of the contest may be a violation of criminal and civil law, and, should such an attempt be made, Eagle Point Advanced Dental reserves the right to seek damages from any such person to the fullest extent permitted by law. Failure to enforce any term of these Official Rules shall not constitute a waiver of that provision.

8. RELEASE AND LIMITATIONS OF LIABILITY By participating in the contest, entrants agree to release and hold harmless Eagle Point Advanced Dental, its officers, directors, employees, and agents (the “Released Parties”) from and against any claim or cause of action arising out of participation in the contest or receipt or use of any prize, including, but not limited to: (a) unauthorized human intervention in the contest; (b) technical errors related to computers, servers, providers, or telephone or network lines; (c) printing errors; (d) lost, late, postage-due, misdirected, or undeliverable mail; (e) errors in the administration of the contest or the processing of entries; or (f) injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from entrant’s participation in the contest or receipt of any prize. Entrant further agrees that in any cause of action, the Released Parties’ liability will be limited to the cost of entering and participating in the contest, and in no event shall the Released Parties be liable for attorney’s fees. Entrant waives the right to claim any damages whatsoever, including, but not limited to, punitive, consequential, direct, or indirect damages.

9. DISPUTES: Except where prohibited, entrant agrees that any and all disputes, claims, and causes of action arising out of, or connected with, the contest or any prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by the appropriate court located in the State of Oregon. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, entrant’s rights and obligations, or the rights and obligations of the Eagle Point Advanced Dental in connection with the Contest, shall be governed by, and construed in accordance with, the laws of the State of Oregon, without giving effect to any choice of law or conflict of law rules (whether of the State of Oregon or any other jurisdiction), which would cause the application of the laws of any jurisdiction other than State of Oregon.

10. PUBLICITY: Except where prohibited by law, acceptance of the contest prize constitutes winner’s consent to Eagle Point Advanced Dental’s use of winner’s name, likeness, voice, opinions, biographical information and state of residence for promotional purposes in any media without further payment or consideration. The winner will be required to record a favorable television commercial for Eagle Point Advanced Dental before September of 2012. Failure to do so will disqualify the entrant. In the case that the winner has already received the dental care and final set of LifeStyle Dentures, and then refuses to record a favorable television commercial for Eagle Point Advanced Dental, the winner agrees to forfeit their dentures, returning them to Eagle Point Advanced Dental within 48 hours of refusal.
